Hyperpyrexia (104°F to 106.7°F) is where the picture changes. This range puts significant stress on the body and can affect major organs if prolonged. What about Kids?

For infants and young children, the thresholds are different.

The severity of fever doesn’t reliably predict the severity of illness either. A viral cold can cause 103°F. Some serious bacterial infections cause only mild fever or no fever at all. And fever itself doesn’t call for antibiotics: most fevers are viral, and antibiotics only help when the underlying cause is bacterial.

And in anyone (adult or child), fever paired with warning signs like severe headache with stiff neck (potential meningitis), difficulty breathing, chest pain, confusion, unusual sleepiness, seizures, severe abdominal pain, a rash that doesn’t blanch when pressed, signs of dehydration you can’t reverse at home, or persistent vomiting deserves immediate evaluation.

Timing matters too. Fever lasting more than 3 days in an adult, or more than 5 days total without a clear cause, is worth investigating. So is fever with a rapid weak pulse, low blood pressure, or severe weakness, which can point to sepsis.

Some people warrant a lower threshold. Anyone immunocompromised, on chemotherapy, or with a serious underlying illness should call earlier. So should anyone who was recently in a tropical area (potential malaria or other travel-related infections) or anyone whose fever started shortly after starting a new medication.

These aren’t “wait and see” situations.

Treating a Fever at Home

For most fevers in otherwise healthy adults and older children, home care is enough.

Acetaminophen (Tylenol) or ibuprofen (Advil, Motrin) can reduce fever and improve comfort. Don’t give aspirin to children or teenagers due to the risk of Reye’s syndrome, a rare but serious condition. Follow dosing directions on the label based on age and weight.
